Practice Health Guides — Terms of Service

1. Agreement to Terms

By subscribing to or using the Practice Health Guides service ("Service") provided by PPCDentist, LLC ("Company," "we," "us," or "our"), you agree to be bound by these Terms of Service. If you do not agree to these terms, do not use the Service.

2. Description of Service

Practice Health Guides is an education-based email lead generation platform designed for dental practices. The Service includes customized lead magnet eBooks, opt-in form installation, and automated email nurture sequences delivered to prospective patients on behalf of the subscribing practice.

3. HIPAA Compliance

The Company takes HIPAA compliance seriously. The Service is designed to collect only non-PHI information (name and email address) at the point of opt-in. Subscriber practices are responsible for ensuring their use of the Service complies with all applicable HIPAA regulations. A Business Associate Agreement (BAA) is available upon request.

4. Fees and Payment

The Service is offered on a monthly subscription basis following a one-time setup fee. Fees are billed monthly and are non-refundable except as expressly set forth herein. The Company reserves the right to change pricing with 30 days' written notice.

5. Cancellation

You may cancel your subscription at any time with 30 days' written notice. Upon cancellation, your access to the Service will continue through the end of your current billing period. No prorated refunds will be issued for partial months.

6. Intellectual Property

All eBook content, email templates, and other materials created by the Company for the Service remain the intellectual property of PPCDentist, LLC. You are granted a limited license to use these materials solely in connection with your subscription to the Service.

7. Limitation of Liability

The Company shall not be liable for any indirect, incidental, special, or consequential damages arising out of or relating to your use of the Service. Our total liability shall not exceed the fees paid by you in the three months prior to the event giving rise to the claim.
